1. ACKNOWLEDGMENT AND ASSUMPTION OF RISK
By signing this document, I acknowledge that participating in or observing force-on-force tactical scenarios, non-lethal training ammunition (NLTA) exercises, and related activities involves inherent risks of serious bodily injury, illness, permanent disability, paralysis, or death.
I explicitly understand and acknowledge that risks include, but are not limited to:
Kinetic impact from marking projectiles (e.g., Speer Force on Force 9mm cartridges), causing bruising, welts, lacerations, eye injury, or soft tissue trauma.
Slips, trips, falls, collisions with structural obstacles, barricades, or other participants.
Equipment malfunction, heat exhaustion, dehydration, or cardiovascular strain.
I freely, voluntarily, and knowingly assume all such risks, both known and unknown, even if arising from the negligence of the Releasees (as defined below) or others, and assume full responsibility for my participation.
2. COMPLIANCE WITH SAFETY RULES & RSO AUTHORITY
I agree to strictly abide by all posted field rules, safety briefings, and directives issued by Range Safety Officers (RSOs) and event staff. Specifically, I agree to:
Mandatory PPE: Wear sealed, ANSI Z87.1+ / MIL-PRF-32432A rated full-face protection at all times when crossing past the cold line or inside active field zones.
Zero Mask Removal: Never lift, adjust, or remove eye/face protection while on the active field. I understand that doing so results in immediate ejection and forfeiture of participation without refund.
Muzzle & Contact Rules: Adhere strictly to the 180-degree muzzle rule, refrain from blind firing, and maintain a minimum 3-foot physical separation from all opponents.
3. RELEASE OF LIABILITY & INDEMNIFICATION
In consideration for being permitted to participate in the event, I, for myself, my personal representatives, assigns, heirs, and next of kin, hereby:
RELEASE, WAIVE, AND FOREVER DISCHARGE [Insert Event/Company Name], its owners, officers, directors, employees, agents, RSOs, volunteers, sponsors, and venue owners ("Releasees") from any and all liability, claims, demands, losses, or damages caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise.
AGREE TO INDEMNIFY AND HOLD HARMLESS the Releasees from any litigation expenses, attorney fees, loss, liability, damage, or cost they may incur as a result of any claim brought against them arising out of my participation or presence at the venue.
4. EMERGENCY MEDICAL AUTHORIZATION & RELEASE
In the event of an injury, illness, or medical emergency during the event:
I authorize event staff, certified on-site medics, or emergency medical personnel to administer necessary first aid, CPR, AED, or emergency medical treatment.
I consent to my transport to a hospital or urgent care facility via ambulance or emergency transport if deemed necessary by on-site medical staff or RSOs.
I acknowledge and agree that I am solely responsible for all medical costs, ambulance fees, and hospitalization expenses incurred on my behalf, and that the Releasees bear no financial responsibility for my medical care.
5. PARTICIPANT ACKNOWLEDGMENT & SIGNATURE
I certify that I am at least 18 years of age, physically fit to participate in high-intensity force-on-force training, and have read this entire document. I understand that I am giving up substantial legal rights by signing it, and sign it freely and voluntarily without any inducement.
